*MUST Complete Online & Update YEARLY

MEDICATION AT SCHOOL

*MUST have the School Medication Form filled out (may print off school website) by Parent/Guardian AND Doctor PRIOR to medication being administered at school

*Over-the-Counter Medications REQUIRE a Doctor’s order, unless approved by the Nurse & Principal

*An ADULT MUST bring in the medication, NOT a Student 

*A NEW medication order from the doctor is REQUIRED each school year  

*ALL medications MUST be in the ORIGINAL bottle to be accepted at the school 

*I ENCOURAGE Parents/Guardians to bring in the Doctor Order/Completed School Medication Form AND the Medication to me @ Open House 

FOOD ALLERGIES

*If your child has a Food Allergy that requires a food substitution - a DOCTOR Note is REQUIRED every year 

IMMUNIZATIONS: K, 7th graders, 12th graders, & New to District

*DUE by the 15th day of School - if NOT obtained/in process your child can be EXCLUDED from school & sports UNTIL obtained

*Bring COMPLETED Immunization Records to me @ Open House 

*Please CALL to make an appointment (Doctor Office, Health Dept., or Local Pharmacy)

VISION & HEARING SCREENINGS 

*Ohio Dept of Health REQUIRES K, 1, 3, 5, 7, 9, & 11 to be screened every year 

*Screenings & Referrals are completed by Mrs. Skinner RN, BSN

*IF your child is REFERRED & you NEED financial assistance - PLEASE contact Mrs. Skinner @ 937-448-2811 ext 1103
